Completing these forms allows me to provide safe, effective, and individualized bodywork for each horse. The detailed information gathered helps me tailor each session to your horse’s specific needs, avoid contraindications, and ensure appropriate collaboration with veterinary care when necessary. Clear communication through these forms ensures mutual understanding and protection for both you and your horse.

What forms do I need to complete?

Before your first session with Flying B's Integrated Equine Therapy, you'll need to complete the following forms:

New Client Intake & Equine History Form: This form collects owner contact information, horse details, discipline, current workload, medical history, injuries, and specific areas of concern.

Consent & Liability Waiver: This document outlines client acknowledgment and consent for equine massage and bodywork services, including a liability release.

Veterinary Clearance (when applicable): Required for certain conditions or post-injury cases, this confirms that massage and bodywork is appropriate for your horse. We also maintain Ongoing Session Notes & Progress Tracking to document findings, treatment focus, and recommendations for every session.

Why are these forms important?

The intake and equine history forms give important insight into your horse’s health background, past injuries, current workload, behavior, and any areas of concern. This information helps tailor each session to your horse’s specific needs, avoid contraindications, and work appropriately alongside veterinary care. Consent and liability forms ensure clear communication, client understanding, and mutual protection for both horse and owner.

How do I submit my completed forms?

Once completed, clients can submit forms electronically by emailing a copy to Flyingbintegratedequinetherapy@yahoo.com or by bringing a completed copy to their appointment. All forms must be completed prior to the first session to ensure adequate time for review. If there are any questions, clients are encouraged to reach out before the scheduled appointment.

Tips for completing your forms efficiently

To help your appointment run efficiently, please complete all forms as thoroughly as possible before your scheduled session. Here are some helpful tips:

Use your horse’s registered name and barn name, plus age, breed, and primary discipline (performance, trail, rehab, etc.).

List any known medical history (past injuries, surgeries, arthritis/PPID/laminitis, ulcers, chronic soreness, etc.).

Include current medications, supplements, and recent treatments (vaccines, injections, chiropractic, dental, farrier work, saddle fitting, etc.), including approximate dates if known.

Describe your main concerns clearly (example: “short striding LF,” “won’t pick up left lead,” “girthing sensitivity,” “right hind stiffness,” “won’t stand for farrier,” etc.).

Note any behavior or handling challenges (ear shy, reactive to flank/hips, doesn’t like legs handled, anxious, history of kicking/biting, etc.). This helps keep everyone safe.

Share your horse’s current workload (days per week, type of work, intensity, recent time off, upcoming competition schedule).

Tell me what has changed recently (new saddle, new rider, new footing, trailer trips, new turnout situation, recent slips/falls).

If you’re unsure about a question, write “unknown” rather than leaving it blank.

Please sign and date all consent/waiver sections.

Before your appointment: Ensure your horse is catchable, reasonably clean/dry, and in a quiet area. Please have a halter/lead ready and a handler available if needed. Avoid distractions during the session (no dogs/children/loud activity), and please do not touch or play with the horse during the session so the horse can fully relax.
